Although the "Keep It With You (KIWY) Personal Medical Information Form was designed for people who need care during disasters and similar situations, the tool helps individuals maintain concise personal medical records by listing medications, medical conditions, and other health related information. It is important for health care workers to have a simple and reliable way to learn information about past and new health concerns for people receiving help.

Founded in 2000 by Doreen Wise after the four-month disappearance and tragic loss of her son, Gabriel, the organization is dedicated to finding the missing quickly and providing support to their families. Building upon our founder's desire to offer help to missing children and their parents, we have recently expanded into the rapidly growing field of Alzheimer's patients who wander from their caregivers.
